For US citizens, upload an image of your valid, non-expired passport or state driver’s license. For non-US citizens, you must upload an image of your valid, non-expired passport.

You must upload a copy of your valid (non-expired), government-issued photo ID with your application. This ID must include your birth date.

Acceptable forms of photo identification for US citizens are:

  • a driver's license,
  • a state-issued photo identification card,
  • or a United States passport.

The name on the photo ID must match the name as it is entered above exactly. If you do not have a valid photo ID that matches the name listed on this form, you must include proof of the name change, such as a marriage certificate or documentation of a court-ordered name change as part of the included upload.

The image must be in at least 200dpi resolution, in color, and in JPEG or PDF format.

Badge Photo

If you expect to receive a badge while working at a Little League tournament, you may choose to upload a headshot you would like to be used on your badge.

If you choose to not upload a badge photo or are unsure if one is needed, you can wait to have a photo taken onsite should it be necessary.

AS A CONDITION OF BEING PART OF THE LITTLE LEAGUE INTERNATIONAL TOURNAMENT, I give permission for Little League Baseball, Incorporated to conduct background check(s) on me now and as long as I continue to be active with the organization, which may include a review of sex offender registries and child abuse and criminal history records. I understand that, if I participate, my position is conditional upon Little League Baseball, Incorporated receiving no inappropriate information on my background. I hereby release and agree to hold harmless from liability Little League Baseball, Incorporated, the officers, employees, and volunteers thereof, or any other person or organization that may provide such information. I also understand that, regardless of previous appointments, Little League Baseball, Incorporated is not obligated to appoint me to a position.
